The small follow-up question most often left behind

Many policy forms use a gate: answer “yes”, and a set of narrower prompts becomes relevant. If the gate changes during a later conversation, the answers beneath it can remain blank.

Trace every changed answer downward

When a response changes, read to the end of the section rather than stopping at the corrected box. Look for requests for dates, duration, treatment, time away from work, current status, and practitioner details.

Then search the rest of the form for the same subject. Travel may connect with occupation; sport may connect with injury; medication may connect with a health event described elsewhere.

Blank does not always mean “no”

A blank can mean overlooked, not applicable, unknown, or to be confirmed. Another reader cannot know which. If information genuinely cannot be recalled, that uncertainty should be discussed with the adviser or insurer rather than disguised as a completed answer.

The applicant remains the source of truth. A consistency reviewer can point out the incomplete branch but should not fill it from assumption.
